A Commonwealth Government initiative established to be the central agency collating information of agencies and programs providing aged care and disability services.
Provides information about community aged care, disability services and other support services including:
- the range of services available and how to contact them;
- who is eligible to receive the services;
- whether there are any costs associated with receiving the services;
- aged care assessment services for access to community aged care packages; and
- aged care assessment services for entry into aged care homes.

The program provides home and community care services to frail aged seniors, people with disabilities and their carers whom reside within Cambridge.
Program include:
- centre-based activities
- home care services
- domestic assistance
- personal care
- social support
- shopping assistance
- home maintenance
- respite
- delivered meals.
Eligibility for services is determined on assessment and a fee for service applies. The service may also assist in determining other support services and information such as maintenance, gardening, transport and accommodation.
